When creating a birth plan, you want to start thinking of what your ideal birth looks like 
prior to the last few weeks of pregnancy.  It is always best to research and know all your options. 
Talk through with your medical and support team ahead of time. 
 
Here is a sample Birth Plan with questions to think about and discuss with your 
provider prior to birth.  *It is a great idea to pre- type your information and wishes. Bring a few copies to the hospital/birth center so those that may be involved in your birth can read through upon arrival and can best help support and fulfill your birth wishes.
